How can I schedule an appointment? 

 

You can schedule an appointment by calling our office at (888) 476-7933 or by filling out the online form here. 

 

What should I bring to my appointment? 

 

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. To ensure the most effective visit please bring the following with you: 

 

 

  • Health insurance cards 

  • Photo ID 

  • Your primary care doctor’s contact information 

  • A list of any medications you are currently taking (prescription and over-the-counter) 

  • A referral from your primary care doctor (if required by your insurance carrier) 

  • Any previous x-rays, MRIs or test results relating to any past foot health conditions 

 

 

 

When should I arrive for my appointment? 

 

For new patients, we suggest arriving 10 minutes in advance to your scheduled appointment to allow adequate time for you to complete any necessary paperwork. 

 

What if I need to cancel my appointment? 

 

We understand that appointments may need to rescheduled or canceled at times. We ask that you notify us as soon as possible so that we can work to reschedule or answer any immediate concerns you may have.
